Planning on using your Insurance? Please start here before booking.
I believe families should understand their insurance coverage before beginning nutrition care whenever possible. Nutrition benefits can vary significantly between insurance companies — and even between plans offered by the same insurance company.
Shae Foudy LLC currently accepts HMSA, HMSA Quest, HMAA, and UHA. However, being a participating or approved provider does not automatically mean that nutrition services are covered under every plan or for every medical condition.
Please complete the following steps BEFORE our initial consultation:
Step 1 — Request an Appointment & Provide Your Insurance Information
If you would like to use insurance begin by requesting an appointment through my secure client portal. During this process, you will be asked to provide your insurance information and a copy of your insurance card.
Please provide complete and accurate insurance information so that coverage requirements can be reviewed before your appointment.
Step 2 — Referral & Medical Documentation
Some insurance plans and medical nutrition therapy benefits require a qualifying medical diagnosis, referral, prior authorization, or supporting medical documentation.
Prior to our appointment please have your child's pediatrician or healthcare provider fax me the following:

Relevant laboratory results

Medication and supplement list

Other records relevant to the nutrition concern

Step 3 — Verify Your Nutrition Benefits
Please download and complete the Verification of Medical Nutrition Therapy Benefits Form below. Use this form when calling your insurance company to verify your benefits. Submit this completed form to the secure client portal once completed. You will be able to access the secure client portal after requesting an appointment to submit the document.
Step 4 — We Review Your Coverage
Once your insurance information and required medical documentation have been received, our office will review the information available to determine whether we can reasonably proceed with insurance billing.
Once your insurance review and all of the previous steps are complete, your appointment will be approved. :)

Important Information About Insurance
Verification of insurance benefits is not a guarantee of payment. Insurance companies make the final determination regarding eligibility, medical necessity, covered diagnoses, authorization requirements, visit limits, and payment when a claim is processed. You are responsible for any deductible, copayment, coinsurance, non-covered service, or other patient responsibility assigned by your health plan in accordance with your financial agreement.
If your plan does not cover nutrition services for your child's needs, you may still choose to receive care using our self-pay option.
Prefer to Self-Pay?
You do not need to use insurance to work with a Registered Dietitian. Families who prefer not to use insurance or whose plan does not cover nutrition services may schedule using our self-pay option.
