Shae Foudy LLC provides individualized pediatric medical nutrition therapy to Hawai‘i families via secure Telehealth. Collaborating with referring providers ensures continuity of care and supports improved health outcomes for our shared patients.

Patients without insurance coverage or with out-of-network plans are welcome to schedule using self-pay rates:
Initial Assessment (up to 90 minutes): $150 and Follow-Up Assessments (up to 60 minutes): $100
How to Refer

Please include the medical diagnosis and corresponding ICD-10 code(s) documented in the patient's medical record whenever applicable.
Because nutrition benefits vary by insurance plan and diagnosis, a referral alone does not guarantee insurance coverage. Providing complete diagnostic and clinical information helps our office appropriately review insurance benefits before the family schedules care.
*If your patient has HMSA, covered diagnoses for medical nutrition therapy only include certain conditions, some requiring pre-certification. View HMSA’s policy of covered diagnoses here.
*If your patient has HMSA Quest, it will only be covered unless prescribed by a licensed physician and determined to be medically necessary.

What Happens After You Refer?
Once the referral and supporting documentation are received, our office will review the patient's information and applicable insurance requirements.
The family will then be contacted with next steps for insurance verification and scheduling.
Nutrition recommendations and relevant updates can be coordinated with the referring healthcare team when appropriate to support continuity of care.
